Chronic care refers to medical care which addresses pre-existing or long term illness, as opposed to acute care which is concerned with short term illness of brief duration. Chronic medical conditions include asthma, diabetes, heart disease, hypertension and depression. Without effective treatment chronic conditions may lead to disability.

The Chronic Care Program at our Centre for Lifestyle Management (CLM) aims to improve patients’ quality of life via affordable care co-ordination and management of chronic diseases to prevent complications and deterioration of health.
